Only appropriate vaccines will be displayed based on your kids age and/or interval from last vaccination.
You need to enter the vaccination history that your kids had. Please enter the information correctly following the record/maternal-child health handbook.
If you want to enter only vaccination history, please choose [Vaccination Record Entry under Reservation Menu.
Please visit our clinic with Pre-Vaccination Screening Questionnaire for Regular Vaccination filled correctly, otherwise it will take time for your registration.
Do not forget to bring Pre-Vaccination Screening Questionnaire for Regular Vaccination and Maternal-Child Health Handbook.
We cannot give vaccination to your kid unless you bring Pre-Vaccination Screening Questionnaire for Regular Vaccination and Maternal-Child Health Handbook
